Guidelines for submitting a “Replacement Diploma Request” form

 

The university offers alumni the opportunity to order a replacement copy of their original George Mason diploma. Payments for the various processing and shipping options to destinations in the U.S. are indicated on the form, and must be made directly to the University Cash OfficePayments will not be accepted by the Registrar’s Office for this serviceOverseas shipping requires an individual price quote.  This may be obtained by contacting the Graduation Services Office prior to submitting the form.

Additionally, the university offers the option to pay for this service using a personal Credit Card; this may be facilitated by completing the "Cash Office Payment Authorization Form", and mailing or taking the completed form along with the "Replacement Diploma Request" form, to the University Cash Office.  The mailing address is located on the form.  Once the appropriate payment has been received and verified by the Cash Office, the "Replacement Diploma Request" form will be forwarded to the Graduation Services Office for final processing.  The amount of time necessary for completion of each diploma order is specified on the form, and is dependent upon the type of service requested.

If you are requesting a name different than what was originally printed on your first diploma, you must submit original documentation indicating a legal change of name; this must be done before your order can be processed.  A diploma processed under this circumstance will bear the following script, in small type face, at the bottom of the diploma: “Reissued following Legal Change of Name”. All “Replacements” will have the script, “Reissued” printed at the bottom.

***All request for replacment diplomas MUST BE NOTARIZED unless signed in person at the Registrar's Office. You will note a section on the form reserved for this purpose.
